Conservation Camp Registration Form
I hereby allow my child to take part in the Dakota Zoo Day Camp program on the grounds of the Dakota Zoo in Bismarck, ND. I further agree not to hold the Dakota Zoo, zoo staff, or any zoo volunteers liable for any injuries that might occur during this program.
I also understand and agree that I will be available, if for any reason, the need arises for my child to be picked up.
